Payment
Registration fees in Euro (€) should be paid as follows. Because of the high costs of credit card payments bank transfers are preferred whenever possible.

a. Bank transfer
All fees must be paid in Euro by bank transfer to Fortis Bank Amsterdam, account no. 24.35.01.234, IBAN nr. NL79FTSB0243501234, and should be made payable to EOS ’05, c/o Eurocongres CONFERENCE MANAGEMENT in Amsterdam. Please clearly state the name(s) of the participant(s). Payments should be made without charges to the beneficiary. If you have registered via the web site, please mention your personal code.

b. Certified/guaranteed payment cheque
The cheque, which should state the participant(s) name(s) clearly, should be made payable to 81st EOS congress 2005 and returned with the completed registration form to: 81st EOS Amsterdam, c/o Eurocongres CONFERENCE MANAGEMENT in Amsterdam.

c. Credit card
The card number, expiration date and CVC should be entered in the appropriate space on the registration form. The CVC are the last three digits to be found on the reverse side of your card, except for American Express  where this is to be found on the front of the card.

Please note:
1 Only the forms of payment listed above are acceptable. Company and personal cheques for which the issuing bank does NOT guarantee payment and Postal Orders CANNOT be accepted.
2 When names of participants are NOT stated on the bank transfer or the cheque, your payment cannot be linked. Consequently, your payment will be
unknown to the administration office.

Confirmation of registration for individual participants
When your completed registration form plus payment or proof of payment is received by the Congress Secretariat you will receive written confirmation of your registration by mail. Registrations received by the Congress Secretariat after 20 May 2005 will not be confirmed in writing. This does not mean that you are not registered. Consequently, please bring the copy of the registration form, that you keep for your own files, as well as the proof of payment of your registration fee with you when you travel to Amsterdam.

Name change
If you should be prevented from attending, you will be given the opportunity to send a colleague in your place. Name changes are possible until 20 May 2005. Should you insist on having the changes carried out after 20 May, there will be a charge of € 40 per name change. Moreover, we will not be able to reconfirm the change to you.

Cancellations and refunds
The Congress Secretariat must be notified in writing of cancellations of registration:

• before 1 April 2005: 70% of the payment will be refunded
• after 1 April 2005: No refunds will be made

Refunds will be handled after the congress.
